Puerto Rico is about a hundred miles across and thirty-five deep. In a week you can drive the whole perimeter, cross the mountains, spend two nights on an offshore island, and still have unhurried mornings. It is one of the most rewarding road trips in the Caribbean, and if you are already flying in for a cruise, the expensive part is paid for.
The short answer
Do it after the cruise if you possibly can. Drive anticlockwise: east coast, then south through Ponce, then the west, then back along the north. Use four or five bases, never seven. Rent a car for the whole week and return it at SJU. Build in one slack day.
Before or after the cruise?
After, almost always. A week-long loop puts you three hours from the port at its furthest point, on mountain roads, dependent on ferries and weather. That is a comfortable trip when the worst case is a changed flight, and a bad idea when the worst case is a missed sailing.
If your dates force the road trip before the cruise, apply one hard rule: finish the loop and be back in metro San Juan a full day and night before you sail. Treat that final buffer night as untouchable. The reasoning is set out on adding days before or after your cruise.
The anticlockwise loop, day by day
Anticlockwise puts the rainforest early, while you are still fresh, and leaves the easy north-coast run for the tired end of the week.

The clockwise alternative
Reverse it and you get the west coast first, which suits you if the surf coast is the main draw or if your flight home is very early and you would rather end near the airport with a short final leg. The trade-off is that El Yunque and the bio bay land at the end of the week, when the weather is a lottery you can no longer reschedule around.
Our preference is anticlockwise: do the weather-dependent things first, so you have days in hand to move them.

Driving realities
- Distances are short, times are not. Nothing on this loop is more than about three hours from anywhere else, but mountain and coastal roads are slower than the map suggests.
- The toll roads are the fast ones. Route 52 south and Route 22 west are the spine of the loop. Ask the rental company how tolls are handled before you drive off.
- Fuel is sold by the litre and stations can be sparse in the mountains. Fill up before you climb.
- Never leave anything visible in a parked car, especially at beaches and trailheads.
- Book the rainforest and the ferries early; both have limited daily capacity.

Build in one slack day
A week with seven fixed plans is not a road trip, it is a schedule.
Leave one day unbooked. Rain will move something, a ferry will change, or you will simply find a beach you do not want to leave. The slack day is what makes the difference between the trip you planned and the trip you enjoyed.
If you have ten days
Add a second island night, a mountain stay in the coffee region, and a slower west coast. Ten days is the point at which you can stop driving every other morning and simply live somewhere for a while. Other shapes are on Puerto Rico itineraries.

Four or five. One night in a place is rarely worth the packing, so double up wherever the driving allows.
